By booking any service or purchasing any item with Revive Glam Studio you are agreeing to these terms. Please understand that it’s your responsibility to read and understand this information because they are non-negotiable.

Sick Policy

If you are experiencing any symptoms of any transmissible virus like covid, the flu, the common cold, stomach bug etc, have been exposed to someone with or any symptoms of the listed, awaiting results for covid, tested positive for covid kindly reschedule.

Cancellation, Late, & Payment Policy

We understand that situations arise for which you may have to cancel your appointment.  As a courtesy to our staff, as well as other clients, we request a 48 HOUR CANCELLATION notice. If it is cancelled less than 48 hours you will be subject to a $70 fee for each 60 minute increment of your scheduled appointment. For any facial waxing service or service 30 minutes or less you will be subject to full payment of your service. 

All appointments require a valid debit/credit card and deposit upon booking each service to secure your appointment. The reservation of an appointment indicates that we have reserved the service time for you and therefore had to decline other customer business. If you do not show for your appointment you will be subject to a $70 fee for each 60 minute increment of your scheduled appointment and banned from booking. For any facial waxing service or service 30 minutes or less you will be subject to full payment of your service. Appointments are considered a no show after 15 minutes of your appointment time has passed. We will process the no show fee 30 minutes after the beginning of your scheduled appointment time has passed and you will be banned from booking in the future. (Please Note: Services 30 minutes or less have a 5 minute grace period and will be rescheduled and charged a no show fee after 5 minutes due to the time frame)

Frequent Rescheduling/cancellations will require a 50% prepayment for services going forward.

There is a 10 minute grace period to make your appointment after that it takes away from your service. After 15 minutes your appointment will have to be canceled and you will be charged a no-show fee. (Please Note: Services 30 minutes or less have a 5 minute grace period and will be rescheduled and charged a no show fee after 5 minutes due to the time frame)

PLEASE NOTE:

Guests who abuse the Revive Glam Studio Policy will be banned from booking.

Refunds

Revive Glam Studio does not offer refunds on any products or services under any circumstance.

Any chargebacks done by guests will result in the guest being banned from booking in the future.

Client Booking, Personal Information, and Agreements

Please be prepared to fill out the necessary paperwork online/in person upon booking your first appointment.

Please share any updated information with the provider at each appointment to ensure we are on the same page during your treatment.

Revive Glam Studio does not share information about clients with anyone unless otherwise noted on the intake form.

Discounts for referrals are only one time use per referral and not applicable unless your name is written on the referred client’s intake form or told to the provider upon check in/out.

Packages and memberships are non transferable or refundable.

Guest’s pictures will not be used without consent on the intake form.

Guests will not be added to any of my communications without consent on the intake form.

Revive Glam Studio reserves the right to deny service and/or call authorities if need be.

Cash app, zelle, venmo or any other, if it’s kind, is not accepted as a form of payment unless a valid credit/debit card is documented.

New clients must leave a credit/debit card on file whether appointment is booked online, in person, or over the phone.

Revive Glam Studio is a cashless spa.
